You can schedule an appointment at any of our convenient CMHS locations, and walk-ins are always welcome.
How to Schedule Your First Appointment
To book your visit, just call us and one of our friendly team members will help you choose the CMHS location that best fits your schedule and needs. During your call, our team may:
- Review your income / insurance information
- Confirm your current address
- Provide a checklist of documents to bring
- Ask a few questions to determine eligibility for any CMHS discount, sliding fee or specialty programs
What to Bring to Your Appointment
Whether you’re a new patient or returning, please bring:
- Photo ID
- Proof of address (ID with current address, utility bill, or other residency document)
- Insurance card(s) (including Medicaid/Medicare, if applicable)
- List of current medications
- Proof of income (recent pay stubs, tax return, benefit statements, employer letter for cash-paid income)
- Co-payment or Sliding Fee payment
Arrival Time
- New patients: Please arrive 30 minutes early to complete registration.
- Returning patients: Please arrive 15 minutes early to update your information.
About Our Sliding Fee Scale
As a Federally Qualified Health Center (FQHC), CMHS offers a Sliding Fee Discount Program based on family size and income.
